Assist – Life Stage, Transition & Support Coordination
We provide capacity-building support to assist participants during significant life transitions. Our approach helps participants navigate change, develop independence, build confidence and connect with appropriate services and community resources.
Key Focus Areas
- Life Transition Planning: Support when moving from school to further education, training or employment; transitioning from the family home to independent or supported living; and adjusting to changes in personal circumstances, support needs or community participation.
- Skill Development: Building practical skills such as budgeting, cooking, planning, decision-making and managing daily routines.
- Support Coordination: Assisting participants to understand and connect with appropriate local services, housing options, education providers and community resources.
- Mentoring and Guidance: Providing individualised encouragement, guidance and practical support to help participants establish new routines and work towards their goals.
